$1 Million CHART Grant to Target Most Vulnerable Patients, Reduce Readmissions

Lowell General Hospital is rolling out a new initiative that will provide outreach and support to Greater Lowell’s most vulnerable patient populations in an effort to strengthen care and reduce readmissions to the hospital. Earlier in 2015, The Center for Medicaid and Medicare Services (CMS) announced it would cover Low Dose CT (LDCT) lung cancer screening for its beneficiaries.
