Getting Started

To make an appointment with the Diabetes and Endocrine Center, contact your Primary Care Provider to fax a diabetes physician order form to 978-323-0362. Once the completed order is received, we will call you to schedule the requested appointment.

NOTE: We strongly suggest that you call your insurance company and verify your insurance coverage if you will be seen for Diabetes Self-Management Education/Training (DSME/T) or Medical Nutrition Therapy (MNT) as some insurance companies have coverage restrictions. Please remember to call our main office if you have any questions at 978-323-0360.
