What to Bring

Please bring the following forms of identification:

Circle Health employees:

• ID badge

Approved External Healthcare Staff: 

• Government-issued photo ID (i.e., driver's license)
• ID badge from current place of employment as a healthcare worker
• Health insurance card (i.e. HMO or PPO card, etc.)
