Chest and Immunization Center
Lost Your Vaccine Card?
We encourage all individuals who were vaccinated in Massachusetts to request, view, and/or print their own COVID-19 SMART Health card. This will allow those who have received the COVID-19 vaccine to download a digitally formatted certification with a QR Code that verifies their immunization status rather than having to carry around a paper card.
To access your vaccination record, visit My Vax Records (https://myvaxrecords.mass.gov/) and follow a few easy steps.
