Lost Your Vaccine Card?
We encourage all individuals who were vaccinated in Massachusetts to request, view, and/or print their own COVID-19 SMART Health card. This will allow those who have received the COVID-19 vaccine to download a digitally formatted certification with a QR Code that verifies their immunization status rather than having to carry around a paper card.
The Chest and Immunization Center at Lowell General Hospital provides comprehensive services for patients who require immunizations, from children to adults.
We are no longer scheduling COVID-19 vaccine appointments or booster appointments for the general public and are unable to accommodate walk-ins at this time. Please visit: vaxfinder.mass.gov to find a vaccine appointment near you.
