Home Health Foundation Names Dianne Farraher-Smith as Chief Clinical Integration Officer

Diane Farraher-Smith, MSN, MBA, RN, former President of Hallmark Health VNA and Hospice System VP of MelroseWakefield Healthcare and former Interim Executive Director of Circle Home, joins the Home Health Foundation (HHF) Executive Management Committee (EMC) as Chief Clinical Integration Officer.

In this newly developed position, Diane will be responsible for the successful and complete integration of home health and hospice services within the Wellforce System to create a cohesive whole that delivers responsive, reliable and evidence-based clinical services efficiently and cost-effectively. She will also be responsible for the delivery of home-based services across the care continuum including traditional home health and hospice care, services for patients with advanced illness, care utilizing technology extenders such as remote patient monitoring and non-traditional population management provisions while partnering with Wellforce system hospitals, physician groups and the Wellforce Care Plan.  Diane will work collaboratively with EMC and Leadership to continue efforts in change management, streamlining operations and transforming Home Health Foundation, Circle Home and Hallmark Health VNA into one, cohesive, home health and hospice organization.

Subsequent to HHF joining Wellforce on October 1st, Diane worked as a key member of the home health and hospice integration steering committee, sharing her extensive knowledge of healthcare regulations and standards while championing major initiatives that were instrumental in the HHF partnership with Wellforce.

Home Health Foundation (HHF), which is comprised of Home Health VNA and Merrimack Valley Hospice, has joined forces with Circle Home and Hallmark Health VNA in the development a new, regional, home health and hospice post-acute continuum with a relentless focus on transforming home-based care.  Together, the agencies serve 124 cities and towns throughout Northeastern Massachusetts and Southern New Hampshire with six offices and a 21-suite hospice house which together provides care to more than 16,000 patients annually. Additionally, Merrimack Valley Hospice serves the Southern Maine region as York Hospital Hospice in partnership with York Hospital. The agencies play a critical role in the health and welfare of patients and families and are committed to innovation, clinical excellence and personalized care.
