Express Registration Instructions

Welcome to Lowell General Hospital's Online Express Registration!

Please note: In order to pre-register online you must complete the online form at least 24 hours before your scheduled test or procedure or 24 hours in advance of when you plan to arrive at the hospital for non-scheduled tests. To register for a Monday visit, you must complete the form by 5pm on the Friday prior.

We have two locations for Laboratory and Radiology services:

The Lowell General Main Campus at 295 Varnum Avenue, located on the ground floor of the new Dahod Building*
The Lowell General Chelmsford Campus at 10 Research Place on Technology Drive in North Chelmsford

*Laboratory and Radiology services are part of our new Patient Service Center in the Dahod Building, accessed through the first main entrance on the right as you enter the hospital campus. Please take advantage of our new amenities, including valet parking, available Monday-Friday from 7:00am to 7:00pm. There is also parking available in the front parking lot and in the garage if you choose.

Laboratory

Monday–Friday, 5:30am – 7:00pm
Saturdays, 5:30am - 1:00pm

Radiology

Monday-Friday, 5:30am - 7:00pm
Saturdays, 5:30am - 1:00pm

Registration

Monday–Friday, 5:30am – 7:00pm
Saturdays, 5:30am - 1:00pm

Using Online Registration

Please enter your information on the following pages. Fields with a red asterisk are required and must be completed in order for you to be able to submit the form; if these fields are left blank, your registration will not be sent.

Important Note: Please complete ALL of the information on the registration form (required and not required) to insure that we can process your pre-registration in a timely and accurate manner. Incomplete form information may cause a delay in your registration process.

If you are having difficulty or would like to pre-register by phone instead, please call 978-937-6429. Representatives are available Monday–Friday from 7:00am–7:00pm to assist you.
