Do you know your risk for heart disease/stroke?
Make an appointment for a full cardiovascular risk screening, which includes blood pressure, body mass index, body fat analysis, cholesterol, blood sugar and peripheral artery disease. Screenings will be performed by a Registered Nurse.
Print out this Cardiovascular Risk Assessment Tool form, complete it and bring it with you.
Fee: $25
Thirty-minute appointments are available starting at 7:30am and are open until 11:30am on Mondays:
