Sub-Acute/Skilled Nursing Facility (SNF) Rehab

A Skilled Nursing Facility (SNF) or sub-acute rehabilitation hospital cares for patients who are on a relatively short path to rehabilitation before they are able to return home. Many patients who have orthopedic surgery like a joint replacement, will move to a sub-acute/SNF facility after leaving the hospital to continue post-surgical care and rehabilitation before going home.

Your case manager will assist in determining the type of facility that best meets your needs as well as arrange for your transition.
